Forms and Registration

Please note: the Health Packet and Check Out forms are REQUIRED forms that must be completed prior to camp and presented at Check-In.

This Packet includes the following three (3) forms:

  • Individualized Standing Order Form - REQUIRED! Regardless if camper requires medication supervision or not. Every camper must have a completed and signed Individualized Standing Order Form. This form must be signed by parent AND physician. No exceptions. The packet also includes a copy of our Medication Guidelines.
  • Health Form - REQUIRED! A new health form is mandatory every year. As completed physical (within 24 months of the registered camping session) is also required.
  • Check-Out Form - REQUIRED! this form is a legal document allowing designated individuals to check-out a camper. It is used for early check-out and/or check-out at the end of the camp session.

  • Campers must bring all completed and signed forms to Check-In.
  • DO NOT mail or fax the Health Packet and Check Out forms. These forms must be hand delivered on the day of Check-In.
  • Please make copies of ALL forms. This will save you many headaches :)
  • If your camper will be attending more than one camp during the summer, you must provide a copy of the Health Packet and Check Out forms at Check-In for each camp. Sumatanga is unable to reference camper paperwork once the camper's original session has ended.

PLEASE MAKE COPIES OF ALL COMPLETED FORMS! You never know when you may need them. They may get lost, you may need to reference the health form physical for another camp or organization, or you may need the physical section for camp next summer. ALWAYS may copies. Sumatanga is not able to reference camper paperwork once the session is complete.

Camp Sumatanga is awesome! They provide a helpful learning experience about God.  Not only do they focus on godly things, but we have lots of fun! The meetings we had with our groups helped us to get closer to one another and talk about our experiences. That small portion out of the day was my favorite.
